Man Falls -- Fatality

A fatal accident occurred recently on an offshore platform drilling rig when a man fell from the casing elevators.

The man was hoisted to the casing elevators 49 feet above the rig floor for the purpose of inserting a cementing wiper plug in a cementing head. As he stood on the elevators he disconnected the hoist line from his safety belt. He then inserted the cementing plug in the head and started installing the cap on the head. The cap blew off the cementing head and knocked him from the elevators. The fall to the rig floor resulted in fatal injuries.

To prevent a recurrence of this accident the operator is taking the following action.

All personnel have been advised to use safety belts and tie them off when working in elevated areas which are not protected by hand rails.
