Injury During Cementing Operations

A cementing serviceman was suspended above the drill floor at the cementinghead preparing to install the wiper plug to commence cementing operations. Thedriller, who was monitoring the well circulation returns, moved back to thedriller's console and lowered the casing to prevent the casing in the hole fromsticking. The driller did not see the suspended serviceman, and as the casingwas being lowered a valve on the cementing head struck the serviceman resultingin an abdominal laceration.

To prevent a recurrence of this type accident, all personnel were advised tomaintain better communication and to survey the entire drilling area beforemoving pipe in this type of operation.
